NOTE TO PARENTS & STUDENTS: FROM
MIKE JOHNSON (OWNER-WICHITA DRIVING SCHOOL):
ALL DRIVER EDUCATION STUDENTS IN THE STATE OF KANSAS ARE
REQUIRED TO FILL OUT A DE 99 APPLICATION. THE FORMS ARE
COMPLETED ON THE FIRST DAY OF CLASS, AND THEN MAILED FROM THE
DRIVING SCHOOL TO THE STATE OF KANSAS. THEY ARE
TYPICALLY PROCESSED AND RETURNED TO WDS IN 7-10 DAYS. WE
DO NOT SCHEDULE THE DRIVING PORTION OF THE COURSE UNTIL THEY
ARE RETURNED. THE DE-99 CONTAINS A PINK INSTRUCTIONAL
PERMIT AND A GOLD CERTIFICATE OF COMPLETION. IF
THE STUDENT DOES NOT HAVE A CURRENT INSTRUCTIONAL PERMIT, THEY
WILL THEN RECEIVE THE PINK, PAPER IP FROM THEIR INSTRUCTOR ON
THE FIRST DRIVING LESSON. MOST STUDENTS, WHICH
RECEIVE THIS PINK FORM NEED TO EXCHANGE IT AT THE DMV FOR A
PHOTO IP.
THE PURPOSE OF THE DE-99 FORM IS TO: 1)
VERIFY THE IDENTITY OF THE STUDENT, WHICH IS APPLYING
TO OBTAIN A LICENSE. 2)
ESTABLISH THAT THE STUDENT CURRENTLY LIVES IN THE STATE OF KS.
3) IDENTIFY IF
THE APPLICANT IS CURRENTLY TAKING ANY PRESCRIBED
MEDICINE FOR A PHYSICAL OR MENTAL CONDITION, WHICH
MAY REQUIRE A PHYSICIAN'S APPROVAL TO PROCEED WITH THE
DRIVING PORTION OF THE DRIVER ED COURSE.
THE PRIMARY PURPOSE
OF THIS FORM IS TO PROVIDE
PROOF THAT THE STUDENT HAS ENROLLED AND LATER SUCCESSFULLY
COMPLETED THE ENTIRE DRIVER ED COURSE AT A LICENSED DRIVING
SCHOOL, OR HIGH SCHOOL. UPON SUCCESSFUL COMPLETION
OF THE DRIVING AND FULL PAYMENT OF THE COURSE, OUR
INSTRUCTOR WILL GIVE THE GOLD CERTIFICATE OF COMPLETION TO THE
STUDENT OR PARENT. DO
NOT LOSE THIS GOLD
CERTIFICATE,
AS IT ALLOWS THE STUDENT TO LATER RECEIVE A LICENSE AT
THE DMV, W/ NO ADDITIONAL TESTING BESIDES AN EYE TEST.
SOME STATES REQUIRE ADDITIONAL TESTING BEYOND DRIVER
EDUCATION, (I.E. WRITTEN/DRIVING), BUT KS USUALLY DOES NOT.
IF YOU LOSE THIS GOLD FORM, OUR WEST OFFICE CAN MAKE YOU
A LETTER OF COMPLETION TO REPLACE IT (COST IS $10). ...Keep
in mind that the DMV can randomly test or deny any license
applicant, or require testing for other reasons (see
below).

Upon successful completion of this course (8 hrs/class
& 6 hrs/driving), YOU WILL NOT RECEIVE A
DRIVER‘S LICENSE. You will receive the gold Certificate
of Completion . This gold document confirms
you successfully completed an approved course in
driver education -from a driving school licensed by
the State of KS. ….By taking this gold document
to a KS Driver‘s License Office (DMV), one can
receive a KS DL, or the license which you're eligible
for. The only test usually required is an eye
test, but the DMV reserves the right to
randomly test
or deny any license applicant. Some other
examples of
testing include: A MEDICAL OR PHYSICAL
condition, a PASSPORT issue, or if the student has
FAILED DMV TESTING several times, etc...
